How do I submit my story?

Step 1. Please fill out the form to share more about your experience as an allied health provider who works with people with Parkinson's.

Step 2. Email two photos that we can include with your story: a minimum of one of you doing your work or living well. (Photos must be in .jpg or .png format. One must also be a high-resolution image that is a minimum of 600px wide by 400px tall (landscape orientation) so it can be used for the feature image.) Email them to us at blog@dpf.org.
